COVID-19 VACCINATION CLINIC (MODERNA, ages 18+ and PFIZER, ages 12+) Hauppauge

Adult Moderna and Pfizer 12+
First doses, 2nd doses and Boosters.
Moderna vaccines are available for everyone 18 years and older only. Pfizer vaccines are available in adult-sized dosages only to everyone 12 years and older. Third doses and Booster doses are available at this clinic. Please find Third and Booster Dose eligibility information in section below this event.

The Clinic can be found on the south-facing side of the building. Parking is available.
To make an appointment, visit: https://vax4.suffolkcountyny.gov/ OR call 311. Residents who have appointments will have priority. Walk-ins are welcome and will be accepted up to one-half hour before the clinic is scheduled to close. If you do not have an appointment, please expect a longer wait time.
Medical practitioners and emergency medical practitioners will be on-site at all times at each clinic to answer any questions and tend to any needs or concerns. Contact 311 if you have questions about the clinics.
